There are three commonly used incision locations for placement of breast implants during Breast Augmentation: within the breast fold (inframammary crease), around the nipple (peri-areolar), or within the armpit (transaxillary).

The inframammary incision is well-concealed and allows the surgeon the best visualization of your tissues. There is a lower risk of difficulty breast-feeding with this incision. All three types of breast implants — saline, silicone gel, and anatomic silicone gel — can be easily placed through  this incision.  The peri-areolar incision is also well-concealed, but it is can be difficult to place larger silicone implants and anatomic silicone implants through this incision. In addition, there is a higher risk of difficulty with breast-feeding and a potentially increased risk of loss of sensation of the nipple.  Recent studies have also suggested that the risk of capsular contracture, or scar tissue around the breast implant, is higher with use of the peri-areolar incision.  While the transaxillary incision has the benefit of being off the breast itself, it may be visible in some types of clothing, and runs a small risk of injury to important nerves and vessels in the arm. Saline implants can readily be inserted through this incision, but large silicone implants and anatomic silicone implants cannot always be placed with this limited access. Dr. Naidu most frequently uses the inframammary incision, and in selected cases will use the peri-areolar incision. She does not perform trans-umbilical (belly button) Breast Augmentation given the poor access to the breast tissue and the elevated risk of injury to the implant.

Dr. Naidu uses the Keller Funnel™ for the insertion of silicone implants in every case.  The Keller Funnel™ is an implant delivery device which allows the surgeon to place a silicone implant easily through a small incision, while reducing the amount of contact between the surgeon’s hands and the implant.  The Funnel™ has an interior hydrophilic coating which creates a slick surface and a low friction interface.  The implant gently passes through the Funnel and into the breast pocket, limiting trauma to both the patient’s tissues and the implant.
